- Unlike the Sprint, Pecco Bagnaia got off to a better start than his rivals, and managed to keep the lead of the race at the first corner. Jorge Martin didn't start the race badly either, as he already finished the first lap in second position.
- Behind, Franco Morbidelli tries to defend his position compared to Pedro Acosta, but the rookie catches the Italian's GP24 while trying to take third place. An incident that ruined the Sunday of the n°31.
- Marc Marquez got off to a better start than the day before. After five laps of the race, the Spaniard already claims sixth position, behind Brad Binder and Enea Bastianini.
- The start of the seventh lap saw the huge disappointment of Franco Morbidelli, who crashed while defending a podium place. This incident promotes Enea Bastianini into the provisional top-3.
- The rain then arrived at Misano, and the strategies came to shake up the race. Jorge Martin was the first of the leaders to get on his second bike after a quick trip through the pits.
- On the track, it is Marc Marquez who begins to shine. The n°93 wipes out his competitors one after the other, and on the ninth lap, he snatches the lead from Pecco Bagnaia.

Bagnaia deprived of home victory
- Things settle down, and Marc Marquez engages in a battle for victory with Pecco Bagnaia, firmly defending his position on a drying track.
- Enea Bastianini is still in third position, ahead of Jack Miller and Alex Marquez.
- The timed laps continue for the leaders, and Marquez is slowly getting closer to a second consecutive victory in MotoGP, just one week after ending 1043 days of waiting. With seven laps to go, the multiple world champion has a good second's lead over his rival of the day. Bastianini, for his part, has completely dropped out, as he is four seconds behind his garage neighbour.
- The top three will remain unchanged until the finish. Marc Marquez creates a surprise and wins the MotoGP Grand Prix of San Marino.
- Brad Binder holds onto his fourth place, ahead of Alex Marquez who completes the top five. fabio quartararo finds his rhythm on his M1, and places his Yamaha in eighth position. Johann Zarco finishes the race as the best Honda rider, in twelfth position.
- Jorge Martin manages to save the point for fifteenth place, which could prove crucial for the championship at the end of the season.
